Ferritin: More Than Just an Iron Storage Protein — The Inflammation Connection You Need to Know

Most people have heard of ferritin as a marker of iron stores—and that's exactly what it is. Ferritin is a protein that stores iron inside your cells, and measuring it in your blood gives healthcare providers an estimate of how much iron your body has in reserve.
But here's something many people don't know:
Ferritin is also a marker of inflammation.
That means your ferritin level can appear normal—or even high—even if your body doesn't actually have enough usable iron.
Understanding this can make a big difference when you're trying to find the cause of symptoms like fatigue, hair loss, restless legs, poor concentration, or low mood.

Ferritin Has Two Jobs
Ferritin is best known for storing iron, but it also belongs to a group of proteins called acute phase reactants.
When your body is dealing with inflammation—whether from an infection, an autoimmune condition, excess body fat, liver disease, or another health issue—it naturally produces more ferritin.
This is part of your body's normal defence system.
The challenge is that an elevated ferritin doesn't always mean your iron stores are healthy. Sometimes it simply reflects that inflammation is present.
Why This Matters
Imagine you have symptoms that strongly suggest iron deficiency:
Persistent fatigue
Hair loss
Brain fog
Restless legs
Poor exercise tolerance
Low mood
Your ferritin comes back at 90 µg/L.
Many people would be told, "Your iron is normal."
But if you also have inflammation, that ferritin level may be higher than it otherwise would be. In other words, inflammation may be masking an underlying iron deficiency.
This is one reason why iron deficiency can sometimes be missed.
Understanding Ferritin Levels
Every laboratory has its own reference range, but many iron experts use additional thresholds when interpreting results.
Above 30 µg/L: This is generally considered the minimum ferritin level needed to avoid iron deficiency in otherwise healthy adults, although individual needs vary.
Above 100 µg/L: Once ferritin rises above this level, healthcare providers should consider whether inflammation could be contributing to the result, particularly if symptoms suggest iron deficiency.
It's important to remember that a ferritin above 100 µg/L does not automatically mean inflammation is present, and a ferritin below 100 µg/L does not rule it out. Ferritin should always be interpreted alongside your symptoms, medical history, and other blood tests.
What About C-Reactive Protein (CRP)?
Another common blood test is C-reactive protein (CRP), which is also used to look for inflammation.
A significantly elevated CRP clearly suggests inflammation is present.
However, even a mildly elevated CRP—such as around 2 mg/L—may indicate low-grade inflammation, even if it's still reported as being within the laboratory's reference range.
This means a "normal" CRP doesn't always tell the whole story.
Looking Beyond Ferritin
If your symptoms suggest iron deficiency but your ferritin doesn't seem to match how you're feeling, your healthcare provider may order additional tests.
These can include:
Serum iron – measures the amount of iron circulating in your blood.
Transferrin saturation (TSAT) – shows how much iron is available for your body to use.
Total iron-binding capacity (TIBC) or transferrin – helps assess how your body is transporting iron.
Soluble transferrin receptor (sTfR) – a test that is much less affected by inflammation and can be especially helpful when iron deficiency is suspected despite a higher ferritin level.
Looking at the whole picture often provides a much more accurate assessment than relying on ferritin alone.
The Bottom Line
Ferritin is an excellent test—but it isn't perfect.
Because it reflects both iron storage and inflammation, a ferritin result should never be interpreted in isolation.
If you're experiencing symptoms of iron deficiency but your ferritin appears "normal," it may be worth discussing whether inflammation could be affecting the result and whether additional iron studies are appropriate.
Good healthcare isn't about treating numbers—it's about understanding what those numbers mean for the individual sitting in front of you.
