Polycystic Ovary Syndrome (PCOS) is one of the most common hormonal conditions affecting women, yet many of us go undiagnosed for years.
Because symptoms can vary widely from person to person, women are often left wondering whether what they are experiencing is “normal” or something worth investigating further.
If you have been struggling with irregular periods, acne, unwanted hair growth, fertility challenges, or unexplained weight changes, PCOS may be worth exploring with your healthcare provider.
This checklist is designed to help you identify common signs and symptoms associated with PCOS and determine whether further assessment may be helpful.

PCOS Symptom Checklist
Menstrual Cycle Changes
Do you experience any of the following?
☐ Fewer than 8 periods per year
☐ Long or unpredictable menstrual cycles
☐ Missing periods for months at a time
☐ Irregular ovulation
☐ Difficulty tracking your cycle because it changes often
☐ Very light or very heavy periods
Irregular or absent ovulation is one of the hallmark features of PCOS.
Signs of Elevated Androgens (“Male Hormones”)
High androgen levels are common in PCOS and may contribute to symptoms such as:
☐ Excess facial hair growth
☐ Increased body hair growth on the chest, stomach, or back
☐ Persistent acne, especially along the jawline
☐ Oily skin
☐ Hair thinning or hair loss on the scalp
☐ Difficulty managing acne despite skincare treatments
Weight and Metabolic Symptoms
PCOS is closely linked with insulin resistance and metabolic health.
☐ Weight gain that feels difficult to manage
☐ Increased abdominal weight gain
☐ Difficulty losing weight despite lifestyle changes
☐ Strong sugar or carbohydrate cravings
☐ Energy crashes after meals
☐ Family history of Type 2 Diabetes
☐ Elevated blood sugar or insulin levels
☐ High cholesterol or triglycerides
Not everyone with PCOS lives in a larger body. Lean PCOS also exists and can still involve hormonal and metabolic dysfunction.
Fertility and Pregnancy Concerns
Many women first discover they have PCOS while trying to conceive.
☐ Difficulty becoming pregnant
☐ Irregular ovulation when trying to conceive
☐ History of miscarriage
☐ Previous gestational diabetes
☐ Fertility treatment history
PCOS-related fertility challenges involve more than ovulation alone and may also include hormonal, inflammatory, and metabolic factors.
Other Common Symptoms
☐ Fatigue or low energy
☐ Mood changes, anxiety, or depression
☐ Brain fog
☐ Sleep disturbances
☐ Darkened skin patches (especially around the neck or underarms)
☐ Skin tags
☐ Difficulty feeling “balanced” hormonally
It is important to note that not every irregular cycle is PCOS. While PCOS is common, other conditions can cause similar symptoms.
